Navigating healthcare and community-service systems can be overwhelming. Positive Coalition Project's Care Access Specialist Program offers personalized, non-clinical assistance to help individuals identify needs, understand available resources, and connect with trusted providers and organizations.
Our Care Access Specialists can:
- Listen to each person's concerns and priorities
- Identify healthcare and social-resource barriers
- Provide clear information about available services
- Connect individuals with healthcare and community providers
- Coordinate referrals and support follow-up
- Help individuals prepare questions for service providers
- Provide health education and health-literacy support
- Advocate for fair and equitable access to resources
