Care Access Specialist (CAS)

At the Positive Coalition Project, our Care Access Specialists serve as trusted guides who help individuals navigate the healthcare system, access community resources, and overcome barriers that impact their health and well-being.
We understand that finding healthcare, understanding insurance, securing transportation, locating food assistance, or connecting with supportive services can be challenging. Our Care Access Specialists provide personalized, non-clinical support to ensure every individual receives the resources and guidance needed to thrive.

Care Access Specalist Program
The Positive Coalition Project Care Access Specialist (CAS) Program helps individuals overcome barriers to healthcare by providing non-clinical advocacy, patient navigation, and care access support.
Our Care Access Specialist works directly with clients to:
- Connect individuals to healthcare providers and services
- Assist with insurance enrollment and benefits navigation
- Coordinate referrals to Community-Based Organizations, Federally Qualified Health Centers, and other service agencies
- Address transportation, housing, food, and other social needs
- Advocate for equitable access to healthcare resources
- Provide health education and support
- Help clients understand and navigate complex healthcare systems.
